Who is eligible for bariatric surgery?
Eligibility goes beyond weight
Bariatric assessment is more than comparing your weight with someone who has already had surgery. Body mass index, calculated as weight in kilograms divided by height in metres squared, is considered alongside medical conditions and treatment history. BMI does not fully describe fat distribution, physical fitness or anaesthetic risk.
Current professional criteria
The 2022 ASMBS/IFSO guidance recommends metabolic and bariatric surgery at a BMI of at least 35 kg/m², regardless of the presence or severity of associated conditions. It also recommends surgery for people with type 2 diabetes and a BMI of at least 30. Surgery should be considered at BMI 30–34.9 when nonsurgical approaches have not produced substantial, lasting weight reduction or adequate improvement in associated disease.
These scientific recommendations do not mean automatic approval for an operation. Hospital and funding rules can differ, and thresholds may need adjustment for particular populations. The criteria must be interpreted within an individual assessment rather than used to select a procedure without specialist advice.
What is assessed?
The team reviews diabetes, blood pressure, sleep apnoea, heart and liver disease, medicines, smoking and previous surgery. A dietitian assesses nutritional deficiencies and eating patterns. Psychological assessment identifies support needs and realistic expectations. Treated depression does not automatically exclude someone: stability and an appropriate support plan are important.
Surgery may be postponed while an unstable medical condition, substance-use disorder or significant psychological or nutritional problem is addressed. This is preparation for safer treatment, not a judgement about the person. Practical access to supplements, blood testing and long-term appointments also needs discussion.
Age and shared decisions
Chronological age alone is insufficient; frailty, function and overall risk matter. Adolescents require specialist assessment that considers growth, maturity and family support. Current pregnancy changes the treatment plan and timing and requires discussion with the relevant specialists.
Ask what health benefits are realistic, which alternatives are available, and what needs improvement before surgery. A well-informed decision respects the patient’s preferences while considering medical risk and the ability to continue care over many years. Eligibility begins a conversation; it does not replace that conversation.
